HGV & TAXI MEDICALS FOR DRIVERS IN NORTHERN IRELAND includes the eye test

 

At Vitalis Health in Belfast city centre we offer comprehensive taxi and HGV medicals for those qualifying as taxi or HGV drivers.

Medicals cost £100 and are carried out at our central clinic in Howard Street, Belfast.

The cost of your medical includes an eye test.

What you need to bring to your appointment

  1. Medical history Summary from your GP. This can be requested from your GP surgery and is called a ‘GP Patient summary’.

  2. Photo ID - driving license.

  3. Medical Forms: a TLM1 or DLM1 Down load HERE.

  4. Are you diabetic? If so bring your record of recent blood sugars on your glucometer.

  5. Do you wear glasses? then bring your glasses/contact lenses as well as your most recent prescription from your optician.

  6. Do you contact lenses? then bring a container to place them in during the eye test as your eyes need to be tested with and without corrective lenses (nb. it may be easier to wear your glasses).

 
 

HGV OR TAXI Medical Forms YOU SHOULD BRING WITH YOU

  1. For your taxi medical you will need to bring a TLM1 medical form which the doctor will complete. The TLM1 form should be issued by the Passenger Transport Licensing Division (PTLD).

  2. For Lorry or bus drivers you will need to bring a DLM1 medical form which the doctor will complete. This can be downloaded here: https://www.nidirect.gov.uk/publications/group-2-lorry-or-bus-licence-dlm1-medical-report

PLEASE NOTE: THERE HAVE BEEN SHORTAGES OF BOTH FORMS.
